Desde su reconocimiento inicial como enfermedad por la OMS en 1948, hasta su actual clasificaci\u00f3n como &#8220;enfermedad cr\u00f3nica compleja&#8221;, su conceptualizaci\u00f3n ha evolucionado considerablemente, generando un debate acad\u00e9mico y cl\u00ednico enriquecedor sobre su naturaleza y abordaje. La propuesta recientemente publicada por The Lancet Commision on Obesity representa una contribuci\u00f3n valiosa a la reevaluaci\u00f3n cl\u00ednica de esta condici\u00f3n. La Comisi\u00f3n The Lancet propone un nuevo paradigma evaluativo multidimensional que complementa el IMC mediante la integraci\u00f3n de par\u00e1metros antropom\u00e9tricos adicionales, mediciones de distribuci\u00f3n adiposa, biomarcadores metab\u00f3licos e inflamatorios, y criterios funcionales y psicosociales. Esta propuesta se fundamenta en un consenso estructurado de expertos multidisciplinarios que definieron 37 declaraciones y recomendaciones con niveles de aprobaci\u00f3n un\u00e1nime o casi un\u00e1nime.<\/p>\n<p style=\"text-align: justify; line-height: 1.8; margin-bottom: 18px; color: #333;\">El objetivo de la presente revisi\u00f3n fue, analizar la propuesta innovadora de The Lancet Commission, examinando sus fundamentos cient\u00edficos, metodolog\u00eda de desarrollo, beneficios para la pr\u00e1ctica cl\u00ednica y salud p\u00fablica, as\u00ed como los retos para su implementaci\u00f3n en distintos contextos sanitarios. La implementaci\u00f3n pr\u00e1ctica de la propuesta de la comisi\u00f3n The Lancet enfrenta desaf\u00edos significativos: limitaciones t\u00e9cnico-log\u00edsticas, costos asociados, necesidad de capacitaci\u00f3n especializada y adaptaci\u00f3n a contextos sanitarios diversos. Since its initial recognition as a disease by the WHO in 1948, to its current classification as a &#8220;complex chronic disease,&#8221; its conceptualization has evolved considerably, generating an enriching academic and clinical debate on its nature and approach. The recently published proposal by The Lancet Commission on Obesity represents a valuable contribution to the clinical re-evaluation of this condition. The Lancet Commission proposes a new multidimensional assessment paradigm that complements BMI by integrating additional anthropometric parameters, measurements of fat distribution, metabolic and inflammatory biomarkers, and functional and psychosocial criteria. This proposal is based on a structured consensus of multidisciplinary experts who defined 37 statements and recommendations with unanimous or near-unanimous approval levels. The objective of this review was to analyze The Lancet Commission&#8217;s innovative proposal, examining its scientific foundations, development methodology, benefits for clinical practice and public health, as well as the challenges for its implementation in different healthcare settings.<\/p>\n<p style=\"text-align: justify; line-height: 1.8; margin-bottom: 18px; color: #333;\">The practical implementation of The Lancet Commission&#8217;s proposal faces significant challenges: technical and logistical limitations, associated costs, the need for specialized training, and adaptation to diverse healthcare settings.
